The written notice we receive on behalf of our client must include:
- Name of the injured
- Location of the hospital
- Amount owed
- Names of potential liable persons
A lien cannot be in an amount that exceeds more than one-half of the damages. A private medical physician is entitled to complete reimbursement without a reduction for possible attorney fees.
